Telehealth Monitoring

Home Care of the Carolinas strives to provide excellent customer service, knowledgeable staff and state-of-the-art equipment, to best meet the needs of our patients and families we serve.

Telehealth helps our nurses and therapists with daily reports, which helps to reduce emergency room visits, physician visits, phone calls to physicians and re-admissions to the hospital.

All of our patients are evaulated for telehealth, with special attention to Congestive Heart Failure, COPD, Diabetes and Hypertension.  The telehealth nurse works closely with the case manager and physician to provide excellent care for the patient.

Advantages of Telehealth Include:

  • Clinical management seven days a week.  Special attention is paid to weight, blood pressure, heart rate and oxygen saturation.
  • Preventative and pro-active care.
  • Most importantly, telehealth allows the patient to take an active role in their health and provides them with the right information and incentive to make healthy lifestyle choices.

Telehealth is a positive tool used in conjunction with our skilled nursing services for better overall outcomes for patients.  Our telehealth equipment is easy for the patient and family to use and is the most clinically accurate monitoring system available.
