Client Responsibilities

  1. Give accurate and complete health information concerning your past illness, hospitalization, medications, allergies and other pertinent items.
  2. Assist in developing and maintaining a safe environment.
  3. Inform the home health care agency when you will not be able to keep a home health care visit.
  4. Participate in development and update of your home health care plan.
  5. Adhere to your development/update home health care plan.
  6. Request further information concerning anything you do no understand.
  7. Give information regarding concerns and problems you have to a home health care agency staff member.
  8. Inform the agency if you have chosen any advance directives.
  9. Provide an opportunity for the primary clinician to review the advance directive and if possible provide a copy to the agency.
