Client Responsibilities
- Give accurate and complete health information concerning your
past illness, hospitalization, medications, allergies and other
pertinent items.
- Assist in developing and maintaining a safe environment.
- Inform the home health care agency when you will not be able
to keep a home health care visit.
- Participate in development and update of your home health care
plan.
- Adhere to your development/update home health care plan.
- Request further information concerning anything you do no understand.
- Give information regarding concerns and problems you have to
a home health care agency staff member.
- Inform the agency if you have chosen any advance directives.
- Provide an opportunity for the primary clinician to review the
advance directive and if possible provide a copy to the agency.
